This is an example of the crib sheet or template that I used when conducting paediatric outpatient clinics in my second placement.

The purple info are things to consider saying, and the stuff in italics are bits and bobs to consider asking about depending on the age of the infant or child in your clinic.

As always, please remember that I am not yet qualified, and that these are just resources that I have created for my own use that I thought other people might also find useful!
